Instructions

To participate in the study.

If you would like to participate in the Ultimate Injuries study, you first need to register. We need to collect some basic contact and health information about you.

You will then receive an email which will have your 4-digit identification number. Please keep this number for entering information later in the study.

There are 3 time periods which will be studied in 2004. Participants can be included in any or all of the studies. After you receive your welcome email, it will include the group number which you have been assigned.

  • Group 1: March 29-May 30 (9 weeks)
  • Group 2: May 31-August 29 (13 weeks)
  • Group 3: August 30-October 31(9 weeks)

Registration Information

    Contact Information: Please be as complete and accurate with this information as possible. The information you provide will be kept confidential and not be used for any marketing or other purposes outside of this study.

Weekly assignment.

Each week you will be asked to log the number of hours you spent practicing and playing ultimate. An email reminder will be sent.

    ID: This is your identification number you were given when you registered for the project. You should also be able to find a copy of the number in the weekly reminder emails.

    Email: Please enter your email address which you registered for the study with. This is to confirm your identity. This information is also contained in the weekly email reminder

    Reporting Period: Select the week that you are reporting the practice/playing time for. The options are scrollable. If you happen to miss a couple reporting periods, you can go back and fill in that data. To maintain as much accuracy as possible, please update the information on a weekly basis.

    Practice time is considered warm-ups and drills. If you did not practice, please enter 0, rather than leave it blank.

    Game time is considered practice games, intersquad games, and tournaments. If you did not participate in a game, please enter 0, rather than leave it blank. Please estimate your actual playing time and not overall time (sidelines, breaks between games) at an event.

    I did not play this week due to a previous injury. Please check this box if your practice and game times were 0 because you were sitting out due to an injury. The previous injury may or may not be due to ultimate. If you were injured playing ultimate, please be sure to fill out the Injury Reporting Form.

Report an injury

Whenever you are injured, simply fill out the Injury Report Form.

    Question #1:
    Practice time refers to warm-ups, drills, stretching, and conditioning.
    Game time refers to practice games or league games.
    Tournament refers to games in tournaments where multiple games are played on the same day.

    Question #5 may be difficult to answer if the injury is new. Please answer with your best guess as to how long you think you will be out. We will keep track of the actual time out on the playing log.

    Question #12 you can list any items you fell may prevent future injuries like you experienced. Examples include better spirit, better fields, proper warm up.

Withdraw from the study
